Butterwick’s Palliative Home Care Team provides care and support in patients’ own homes day or night.
This service can be accessed by any patient with palliative care needs in the Durham Dales and Sedgefield localities. The Palliative Home Care Team (PHCT) is here to support the patient who wishes to remain at home during the palliative stage of their illness.
The PHCT provide senior health care assistants trained in palliative care who support patients and allow carers some time off from their caring responsibilities. The Team is able to provide respite and support for periods of between 3 and 8 hours at a time and can be available day or night.
Any health care professional may refer a patient to the Palliative Home Care Team but the District Nurse must also be aware of the referral and have involvement in that patient’s care.
The aim of the service is to offer high quality care to patients and their families in their own homes and also to prevent unnecessary admissions to hospital and to support the discharge of patients with palliative needs from the hospital or hospice.
Butterwick uses patient service audits to ensure our services are up to and beyond patients’ expectations. These surveys are also used in an effort to improve our services wherever necessary. These are a few comments taken from a recent service user audit:
