Palliative Home Care Team

This service can provide both qualified nurses and health care assistants who have experience in palliative care. A carer can be provided for periods between 3 and 8 hours at a time and can be available day or night.

Any health care professional may refer a patient to the Palliative Home Care Team but the District Nurse must also be aware of the referral and have involvement in that patient's care.

The aim of the service is to offer high quality care to patients and their families in their own homes and also to prevent unnecessary admissions to hospital and to support the discharge of patients with palliative needs from hospital or hospice.
